CHL - Proof of Citizenship

 

The Polk County Sheriff’s Office requires proof of citizenship from all applicants. Once proof is provided we will retain a copy of it in your file and will not require you to provide it again.  If we do not have a file for you then you will be required to provide proof of citizenship in one of the following forms:

 

If you have had your name legally changed AND your new legal name is not on your current and valid Oregon Driver's License OR your Official U.S. Passport, then you must bring in a certified true copy of your legal name change.

 

1) Official U.S. Passport
            (Must not be marked cancelled, be hole-punched or have clipped corners.  Expired passports are still ok for proof of citizenship)

2) Original or Official copy of U.S. Birth Certificate

3) FS-240 Form (Consular Report of Birth Abroad )

4) Recent N-300 Form (Application to File Declaration of Intention) 
            (along with a letter stating that your application has been received & proof of 6 months residency)

5) Original Naturalization Certificate

6) An Official Tribal ID Card from a Federally recognized Tribe
