Report a Crime

The Polk County Sheriff’s Office recognizes the inconvenience of trying to make time to file a police report. Keeping that in mind, this web form was created to allow you to report certain crimes  from the comfort of your own home. View List of Crimes which can be reported online.

 

You may report the crime via this web form IF:

You Have Been a Victim of One of the Online Reportable Crimes,
There are NO SUSPECTS in the Case,
You Need a Case Number for Insurance Purposes, or
You have ten property-related items or less that were affected.

However, if you feel more comfortable speaking with a deputy in person, you can still call our office at (503) 623-9251 to arrange to speak with a deputy.

**If the crime occurred within the city limits of Salem, Dallas, Monmouth or  Independence, please contact that police department instead of submitting this form to the Sheriff's Office.**

NOTE: Initiating a False Report in Oregon is a Class A Misdemeanor.

All boxes and information must be completed in order for the Polk County Sheriff’s Office to generate a case number for your records. If you don’t fill out the form in its entirety, your report will not generate the case number that is needed by your insurance company. If your report is complete, you will be called within a week and provided with a case number.
