Oregon is a closed record state. Polk County is unable to release any information regarding a birth or death record by phone, mail or email to anyone not eligible to receive a certificate of birth or death.

BIRTH AND DEATH CERTIFICATES

We accept vital records requests for birth and death certificates by email, mail, or fax. If you have a request that needs to be expedited, please call our office for assistance. 
Polk County Vital Records provides certified copies of birth and death certificates. Birth and death events must have occurred in Polk County and within 6 months of the birth or death date.                                  

Please note verification of eligibility is required to order and receive a certificate and additional documentation may be needed to verify identity of individual ordering.                         

Processing delays could occur that are beyond our control. 

If an event is past 6 months from the date of birth or date of death, please contact the State of Oregon Vital Records.

HOW TO ORDER:

• Complete appropriate order form 
• Birth Certificate Order Form/español
• Death Certificate Order Form/español
• Provide proof of identity
• Provide appropriate legal documentation          

We ensure you are an eligible recipient to order and receive a certificate of birth or death. Legal documentation will be utilized as proof of eligibility.

Initial fee is non-refundable once we have completed a search for a birth or death record.
