Polk County Public Health is only accepting clients for immunizations by appointment only. No walk-ins allowed at this time. 

 

Polk County Public Health/ Immunizations provides the following vaccines: DTap, Tdap, Td (Tetanus containing vaccines), IPV (polio), Hepatitis A & B, HIB, HPV (Human Papilloma Virus), Influenza, Meningococcal, MMR (Measles/Mumps/Rubella), Rotavirus, Varicella (Chicken Pox), and Pneumoccocal (pneumonia).

TB tests are available. Appointments are necessary. Please call (503) 623-8175 to schedule an appointment. A fee for services, based on your insurance plan, will be charged.

 Vaccines protect your child against many childhood diseases. Immunizations (vaccine or shots) for infants, children and adults are offered at the following Polk County location, by appointment only (503) 623-8175

 

Immunization Records

If you need a request of your child's records please download this form and mail or bring it in to Public Health. If your child is 18 or over 18, they must fill this form out and sign to receive their records for Immunization. 

Travel Immunizations

Information about travel immunizations is available from the Center for Disease Control and Prevention Traveler's Health website.
