What is Service Integration?

The purpose of the Service Integration program is to facilitate collaboration among community partners to provide coordinated resources and information for individuals and families. We are agency and community partners expediting solutions by matching resources to clearly defined needs, while avoiding duplication of service. 

Service Integration is a partnership between Polk County Family & Community Outreach, six school districts, 200+ partnering organizations, agencies, and individuals throughout the county. There are six Service Integration Teams that represent a local school district catchment area and each meet once a month to network, share resources, and discuss local needs. 

Who are We?

There are six (6) Service Integration Teams, one for each school district in Polk County. These teams meet monthly and are open to anyone wanting to learn more about the valuable resources in the communities. Each team is unique to its local community with partners such as school personnel, law enforcement, government (city, county, state) agencies, non-profits, faith community representatives, tribal members, and community members. 

Service Integration Supported Solutions:

In Service Integration, we believe in teamwork! By working together and combining resources, the teams are able to support a variety of local projects and needs each year, including:

Urgent Needs: Emergency housing or utility assistance

Basic Needs: Food, clothing, prescriptions, and personal items

Community Development: Community events and activities

Education: Classes, tuition, fees, literacy programs for adults or children

Extra Curricular Activities: Fees, materials, clothing or shoes needed for participation in extra curricular activities

Youth Development: Leadership/learning opportunities not part of regular school programs.
