Home Visiting Services offered by Polk County Public Health

Polk County Public Health offers two home visiting programs to families in our community: Babies First! and CaCoon. 

Babies First!

Babies First! is a program for pregnant women and for babies/children from 0-5 years old. The goal of Babies First! is to help families make sure that their babies are healthy as they grow and learn. This includes helping pregnant women access resources to have a healthy pregnancy. 

Nurses make home visits to provide health screenings, developmental screenings as needed and referrals to community services. Nurses provide support, encouragement and education on a variety of topics related to pregnancy and/or parenting. For more information about this program click here:

CaCoon

CaCoon is a home visiting program for families with children through the age of 20 who have complex medical conditions which result in special medical, educational, vocational, and social needs. The goal of this program is to empower parents to become their own case managers and to utilize community resources with the support of a nurse.

Nurses make home visits to provide health screenings, developmental screenings as needed and referrals to community services. Nurses provide case management and care coordination to assist families in meeting the complex needs of their child. For more information about this program click here:

Family Connects Home Visiting Network

Polk County Public Health is excited to partner with Family Building Blocks, Mid-Willamette Valley Community Action Agency, Oregon Child Development Coalition, Willamette ESD and other in home service agencies in the Family Connects Home Visiting Network.  This Network has been working together since October 2013 to strengthen families in the Polk County community. Each agency involved in the group offers a type of in home service to families. These services can range from educational curriculums to complex medical monitoring and serve families prenatally through age 5 and up.   The main goal of the Family Link Home Visiting Network is to improve the overall health of the community and improve kindergarten readiness by increasing in home service coverage for Polk County families.   The group has developed a Universal Referral tool to be used by any provider or community member wishing to refer a child or family to in home services. This referral form will be used by the group to aid in placing families with the in home service that best fits each family’s needs.
