Justice Involved Transition Support

Polk County Behavioral Health has a team of service providers who support individuals with behavioral health issues who frequently have contact with law enforcement and the criminal justice system.

Who We Serve

Transition support is provided to individuals who:

Are 18 years old or older
Have a Mental Health Diagnosis
Are involved with the criminal justice system or law enforcement because of a mental health reason
Have recently been released from jail or prison or have an anticipated release date no further than 30 days out
Are willing to engage in treatment
