Wraparound principles

What is Wraparound?

Wraparound is a care planning process for youth with mental health concerns and complex needs. Parents or guardians may request Wraparound Services when counseling has not been enough to safely address serious concerns. 

The Wraparound process begins with identifying the strengths, needs, and culture of the youth and family. The youth and family work with a Wraparound Care Coordinator to form a Wraparound Team. The Team is there to support the youth and family. The Team helps create a plan that focuses on needs and builds on strengths. A team might include friends, relatives, teachers, and other professionals involved in the youth's life. The Wraparound process may take several months or longer. 

The types of service and supports on a plan will depend on individual needs. The setting of where services will take place depends on needs and safety concerns. 

Participation in Polk County Wraparound is voluntary.
 

Polk County Wraparound is driven by 10 principles:

  1. Family Voice and Choice: The family or youth's vision for their life is always considered. Options are given to families so that they can decide what is best for them. 
     
  2. Team Based: Team members (natural and formal supports) are chosen by the youth/family to support them in brainstorming.
     
  3. Natural Supports: The team should include people from the youth/family's personal network.
     
  4. Collaboration: Team members work cooperatively and share responsibility for plans. The plan should be a blend of all the mandates at the table and move in the direction of the youth or family's vision.
     
  5. Community Based: Wraparound aims to be inclusive and accessible with options that will integrate the youth/family into their community. 
     
  6. Culturally Competent: The Wraparound process shows respect for and builds on the values, preferences, beliefs, culture, and identity of the child, youth, or family. 
     
  7. Individualized: The needs addressed and options that youth/families have to choose from should reflect the individual. 
     
  8. Strengths Based: The Wraparound process identifies, builds, and utilizes family and youth strengths that will help the team/family to reach their vision and meet their needs. 
     
  9. Unconditional Care/Persistence: The Team persists in working towards the goals identified. We believe that youth/families don't fail; plans fail.
     
  10. Outcome Based: Goals and strategies are reflected on to ensure that what the team is working on is going to have a lasting impact. 

 

Eligibility

Wraparound Services are a covered benefit for individuals with Medicaid (Oregon Health Plan), including plans managed by Pacific Source Marion Polk CCO and/or Open Card plans. 

Polk County Wraparound is available to youth ages 2-17 years old at admission who meet criteria as described below: 
 

Must meet all 5 of the following

  1. Interested and willing to engage in the Wraparound Program.
  2. Currently, or at risk of multiple agency involvement (DHS Child Welfare, Juvenile Justice, Special Education)
  3. Youth has received a Mental Health Assessment within the last 60 days; or is willing to engage in a Mental Health Assessment with PCBH as part of the screening process.
  4. Has an active Mental Health Diagnosis
  5. Current care coordination needs have not been met by current system of care

-AND-

At least 1 of the following

  • Stable living placement has been disrupted or is at risk of disruption due to behavioral health needs.
  • Frequent or imminent admission to inpatient or intensive mental health treatment services.
  • Significant risk of losing school or day care placement due to behaviors related to mental health needs.
  • Elevated risk that disrupts activities of daily living
  • Family support system and environmental stressors impacting activities of daily living.

Current placement in the following settings

  • Secure Adolescent Inpatient Program (SAIP)
  • Secure Children’s Inpatient Program (SCIP)
  • Psychiatric Residential Treatment Service Program (PRTS)
  • Commercially Sexually Exploited Children’s Residential Program

 

Access

Current clients of Polk County Behavioral Health may request a referral for Wraparound Services by speaking with their therapist.

For new inquiries, and to request a screening for PC Wraparound, complete PC Wraparound Request Packet and submit it to Polk County Behavioral Health.
