Polk County Community Health Improvement Plan 2012
We are pleased to present the Polk County Community Health Improvement Plan (CHIP). This plan is a combined effort of dedicated community partners who collaborate together as the Polk County CHIP work group. The CHIP takes an in depth look at factors negatively affecting the health of the Polk County community and provides solutions to combat their detrimental effects.
