ADULT INTENSIVE SERVICES TEAM

Polk County Behavioral Health’s Adult Intensive Services are intended to support individuals who meet criteria for a higher level of care through community based services. Our team is made up of Qualified Mental Health Associates who provide case management and skills training, empowering individuals to realize their full potential and to reach recovery.
 

Assertive Community Treatment (ACT)

This is an evidence-based treatment model for individuals meeting criteria for the highest tier of intensive services. The ACT team provides wraparound services for adults with serious mental illness who are most at risk of state hospital admission or who are stepping down from a psychiatric hospitalization. Services are tailored to meet the specific needs of the individual and assist with recovery, rehabilitation, and reintegration.
