Online Funding Request To request funding from an SI Team, a completed request form must be submitted at least one day prior to the next regularly scheduled meeting date 

Service Integration

The purpose of the Service Integration program is to facilitate collaboration among community partners to provide coordinated resources and information for individuals and families. We are agency and community partners expediting solutions by matching resources to clearly defined needs, while avoiding duplication of service. 

Expunction

ORS 419A.260 through 419A.265 provides for the removal and destruction of many police and Juvenile Court records and a written explanation of your rights. You may be eligible to have your juvenile record expunged if:

 

A. You were NEVER found within the jurisdiction of the juvenile court, and you meet ALL FIVE of these conditions:

1. You are at least 18 years old.

Emancipation

The Polk County Juvenile Department will assist you with the emancipation process.

Juvenile Traffic, Boating or Game Law Offenses

Polk County Courts has a standing order of automatic waiver of juvenile traffic, boating or game law offenses in place.

Sanction Court

Sanction Court is our primary diversion program.

Polk County Public Health/ Immunizations provides the following vaccines: DTap, Tdap, Td (Tetanus containing vaccines), IPV (polio), Hepatitis A & B, HIB, HPV (Human Papilloma Virus), Influenza, Meningococcal, MMR (Measles/Mumps/Rubella), Rotavirus, Varicella (Chicken Pox), and Pneumoccocal (pneumonia).
