In order to provide proof of residency, you must have the most up-to-date information on your Oregon Driver’s License or Oregon Identification Card and they must be valid.
You may also provide one of the following:
DMV receipt showing your name and change of address
Current rental or lease agreement
Current voter registration card
* People who travel & have no permanent residence, but consider Polk County as home may use this
You can also check online with different County Circuit Courts, have an attorney produce the legal forms you need or check with stationary stores that carry Do-It-Yourself legal forms.
