COMMUNITY HEALTH ASSESSMENT
Polk and Marion Counties in partnership with local health professionals and community organizations conducted a Community Health Assessment (CHA). The 2019 CHA provides most current demographic, socioeconomic and health data for Polk County.
This information will be used to create a Community Health Improvement Plan (CHIP) to focus efforts on key priorities.
