Polk County Family & Community Outreach / Resource Center has moved!

The Polk County Family & Community Outreach Department (FCO) has moved! FCO is excited to announce that all programs have moved to the new location in Monmouth behind the Roth's Fresh Market, and the Resource Center will now be known as the Polk Community Resource Center. The new address is 1407 Monmouth Independence Highway, Monmouth, OR 97361, and the main office number is 503-751-1644. FCO's typical office hours will be Monday through Friday, 8 am - 5 pm, closing for lunch each day from 12 pm - 1 pm.

The previous location of FCO was at the Academy building in Dallas, with a satellite office in Monmouth at the Annex across from Ash Creek Elementary.

The Monmouth location will now house Service Integration, Mid-Valley Parenting, Drug, Alcohol, School-Based Mental Health, and Program Gambling Prevention. The Resource Center will continue to provide rental and deposit assistance, food, hygiene supplies, and connections to other resources across Polk County. More local services will co-locate with us beginning in mid-January. Stay tuned for details.

Please note: The Polk County Veteran Service Office will remain in Dallas until January 10th, then move to the Monmouth location.
