You have the right to file a complaint about the services you received from: 

• Your local office
• Brokerage
• Community Developmental Disability Program
• Children's Intensive In-Home Services
• Other paid provider

You can file a complaint by completing the DD Services Complaint Form (or see alternate versions attachmented below)  or on the State of Oregon/DHS-Senior & Disabilities website.
