Polk County Voter Records Request Form

General Information:

It is Polk County's policy to respond to voter records requests in an orderly, consistent and reasonable manner.  The above form needs to be completed with name, contact information, and a clear indication on what records are being sought.

The Compensation Committee with be meeting at 9:30 am, located in the main conference room at 850 Main Street, Dallas, in order to discuss the compensation wages for Elected Officials for FY2025-2026. This meeting is open to the public.

• April 21st:  Out of state ballots mailed
• April 29th:  Voter registration deadline for new and canceled voters
• May 1st:  Ballots mailed to voters (drop-sites open)
