Every three years, Polk and Marion Counties in partnership with local health professionals and community organizations conduct a Community Health Assessment (CHA). The last assessment was completed in 2019 and it provides demographic, socioeconomic and health data for Polk County. Each year the CHA is updated with the most current information.
This information will be used to create a Community Health Improvement Plan (CHIP) to focus efforts on key priorities.
