FREE Mobile Breast Cancer Screening

The Polk County Public Health Department is partnering with the Ford Motor Company Fund to host a FREE mobile breast cancer screening center on December 9th in the Academy building parking lot located at 182 S.W Academy St in Dallas, OR 97338.

The ultrasound screenings are by appointment only and usually take less than 30 minutes.  The test results will be mailed to your house 4-6 weeks after your appointment.

Polk County Public Health is only accepting clients for immunizations by appointment only. No walk-ins allowed at this time. 

 

Polk County Public Health/ Immunizations provides the following vaccines: DTap, Tdap, Td (Tetanus containing vaccines), IPV (polio), Hepatitis A & B, HIB, HPV (Human Papilloma Virus), Influenza, Meningococcal, MMR (Measles/Mumps/Rubella), Rotavirus, Varicella (Chicken Pox), and Pneumoccocal (pneumonia).

WIC (Women, Infants, & Children)

The WIC program provides families with nutritional education and counseling, nutritious supplemental foods, and health screenings.
