COMMUNITY SUPPORT SERVICES

Our team of Qualified Mental Health Professionals and Associates utilize the “Illness Management and Recovery” model to support individuals struggling with severe mental illness. Services focus on recovery, independence, and self-sufficiency. Services provided or overseen by our team include:

Case Management

Mental Health case management is available to all individuals, when referred by their therapist or prescriber, to help them through periods of unusual difficulty or stress in their lives. Case management provides a blanket of comprehensive services to help individuals remain stable and independent in the community, reducing the instances of disruptions to daily life, and reducing the risk of hospitalizations.

Our team of Qualified Mental Health Associates collaborates with other community social service agencies, families, therapists and the individual to leverage and make certain there is efficient and effective use of resources while assuring the individual's needs are met.
 

Individual and Group Therapy

Qualified Mental Health Professionals in our Community Support Services Team may work with clients in individual and group settings. These individuals are usually receiving case management services already. Their therapy goals typically focus on achieving and maintaining personal goals toward recovery, self-sufficiency, and independence.
 

Peer Support Services

Peer Wellness Specialists provide support to individuals coping with mental health and/or physical wellness challenges by sharing their experiences, strengths, and recovery in individualized or group visits.
 

Assertive Community Treatment

Assertive community treatment targets individuals who are at risk of harm to themselves or others because of their confused psychotic thinking. These individuals may have difficulty meeting their own basic needs.  

Our team provides flexible, non-clinic based individualized services to this population. This team develops a crisis plan and an individualized support plan. The crisis plan identifies expectations on how to be treated during a relapse. The individualized support plan includes things such as daily medication management and case management.
 

ECOS (Enhanced Care Outreach Services)

ECOS provides comprehensive Mental Health treatment for individuals with chronic and severe mental illness(es) who meet Senior and People with Disability’s (SPD) eligibility criteria. Individuals must demonstrate a need for intensive medical and psychiatric services, have a history of failed residential placements, and reside in a community setting.
 

Mental Health Residential Placement

Polk County’s Residential Specialist oversees the operation of state-licensed Mental Health residential facilities located within the County. They focus on level-of-care needs in deciding appropriate individual placements in residential settings.
 

Supportive Housing

Polk County has a Medicaid funded 6-unit supportive housing apartment complex with on-site case management services. We also offer supportive housing services for non-Medicaid eligible or transition-age youth who are diagnosed with a serious mental illness living in Polk County.
