Developmental Disabilities

Mission

To guide individuals with Developmental Disabilities in their choice of supports that promote well-being, independence and integration. We believe in helping individuals lead fulfilling lives and reach their potential.

Service Coordination

The Polk County Developmental Disabilities Program offers service coordination to individuals who are eligible for services due to an intellectual disability and/or a developmental disability. This is in accordance with the Oregon Administrative Rule 411-320-0010. It states, "Service Coordination means an organized service to assist individuals to select, obtain, and utilize resources and services."

  • Advocacy

Service Coordination services are offered at no cost to the individual/family. Direct care services (for example, respite, residential and vocational services) are available, as funds are available.
