Mental Health Services provides a 24-hour, seven-day-a-week crisis service for Polk County that includes:

On-Site Services
On-Site services are available Monday through Friday from 8:00 a.m. to 5:00 p.m. at 182 S.W. Academy in Dallas (Google Map) or by calling (503) 623-9289. We are open during the lunch hour. If you need to be seen by a crisis worker, you will need to arrive before 4:00 p.m.
Off-Site Services
Services include:
Emergency evaluations and crisis interventions at local hospital emergency rooms, the Polk County Jail, county schools, and throughout the community at the request of local law enforcement.

Telephone Triage
A resident of Polk County wishing to speak with a crisis worker and/or to engage in services at Polk County Mental Health can call (503) 623-9289 and receive a telephone triage, or come into our Dallas office (182 SW Academy St), to speak with a crisis worker. Hours are Monday -- Friday 8:00 a.m. until 5:00 p.m.
Office Visit
If you decide to come into our Dallas office to meet with a crisis worker face-to-face for an initial crisis screening assessment, you will need to arrive before 4:00 p.m.
