"Connecting Polk County Individuals and Families to Valuable Resources and Services."

What is Service Integration?

The goal of Service Integration is quite simple: "Provide coordinated community resources and information to families and individuals in Polk County."

We do this by meeting with area partners on a regular basis to build relationships, share services and resources, and brainstorm ways to offer assistance and identify gaps in services for those living here.

Who are We?

There are seven (7) Service Integration Teams, one for each school district in Polk County and one Team that shares County lines with a Yamhill County school district. These teams meet monthly and are open to anyone wanting to learn more about the wonderful resources here in our communities. Each team is unique to its local community with partners such as school personnel, law enforcement, government (city, county, state) agencies, non-profits, members of the faith community, service groups, Grand Ronde tribal members and community members not affiliated with any other group or organization.

Teamwork

Here in Polk County, we believe in Teamwork! By working together, Service Integration supports a variety of local projects, activities and needs each year, including:

  • COMMUNITY OUTREACH EVENTS
  • SCHOOL EVENTS AND CURRICULUM
  • SCHOOL SUPPLIES
  • SPORTS FEES
  • PARENTING CLASSES
  • ASSISTANCE FOR INDIVIDUALS & FAMILIES IN CRISIS (Basic Needs)
  • BOOKS FOR CHILDREN
  • COMMUNITY HOLIDAY PROJECTS
  • BUS PASSES...And more!

How Can I Learn More about Resources in Polk County?

The Polk Community Resource Expo (previously known as the Academy Training) will help you gain a broader knowledge of, and appreciation for, the community resources that make up Polk County! Representatives from agencies throughout the County give an overview of what role their agency plays in Service Integration in Polk County in a fun, classroom format. Please check back soon for details on our next session.

Are There Other Ways to Learn about Resource Information in my Community?

You can also add your e-mail address to one or all the Team groups to receive the monthly SIT Comm Newsletter, meeting reminders, notes and urgent resource information. If interested, please submit the online Registration Form or contact the SI office at (503) 623-9664.

How Does Service Integration Impact the Community?

Each Service Integration Team is as unique as the community it serves. Every one of the communities brings its own blend of talents and strengths. When forces join together it is remarkable what can be accomplished.  In the Supporting Documents below, please select the Service Integration Team of your choice to see a Year-End summary of Team activities.
