"Connecting Polk County Individuals and Families to Valuable Resources and Services."
The goal of Service Integration is quite simple: "provide coordinated community resources and information to families and individuals in Polk County."
We do this by getting with area partners on a regular basis to build relationships, share services and resources, and brainstorm ways to offer assistance and identify gaps in services for those living here.
There are six (6) Service Integration Teams, one in each school district in Polk County. These teams meet monthly and are open to anyone wanting to learn more about the wonderful resources here in our communities. Each team is unique to its local community with partners such as school personnel, law enforcement, government (city, county, state) agencies, non-profits, service groups, Grand Ronde tribal members, faith congregations and community members not affiliated with any other group or organization.
Here in Polk County, we believe in Teamwork! By working together, Service Integration supports a variety of local projects, activities and needs each year, including:
The Polk Community Resource Expo (previously known as the Academy Training) will help you gain a broader knowledge of, and appreciation for, the community resources that make up Polk County! Representatives from agencies throughout the County give an overview of what role their agency plays in Service Integration in Polk County in a fun, classroom format. Please check back soon...for details on our next session.
You can also add your e-mail address to one or all of the Team groups to receive the monthly SIT Comm Newsletter meeting reminders, notes and urgent resource information. If interested, please submit the online Registration Form or contact the SI office at (503) 623-9664.
