Marriage Licenses/Partnerships

Marriage Licenses

To obtain a marriage license, each applicant must be at least eighteen (18) years of age (or 17 years old with a parent or legal guardian's consent). Each couple must appear in person.

There is a three (3) day waiting period before a license may be used, and it is valid for sixty (60) days after the effective date. THE LICENSE IS VALID ONLY IN THE STATE OF OREGON.

There are no blood tests required.

The cost of the license is $50.00, payable by cash or check. Credit cards are not accepted at this time.

 

Request for Certified Copies

A certified copy of your marriage license is used to show proof of marriage, or effect a name change with agencies such as Social Security or the Department of Motor Vehicles.

To obtain a certified copy, you may either visit the Polk County Clerks Office or request a certified copy by mail.

The fee is $4.00 per certified copy at the counter, or $7.75 if requested through the mail.

When requesting a copy by mail, please include the following information:  both partners names, and name prior to marriage if different, the date you were married, and an address to mail the certified copy back to. Mail your request to:

Domestic Partnerships

Effective February 4, 2008, same-sex couples wishing to be legally recognized partners in a domestic partnership may register with the County Clerk's office.

Forms may be obtained at:

or at the County Clerk's office.

Requirements
All forms must be printed on legal sized paper -- 8.5 inches by 14 inches -- and completed in blue or black ink.

At least one partner must be a resident of Oregon.

Both partners must be of the same sex and 18 years of age and older.

Both partner's signatures must be notarized by a Notary Public. There are notaries available throughout the building, or you may use an outside notary. Once the form is completed, signed, and notarized, it must be filed at a County Clerk's office in Oregon.

Fees
$50, payable by cash or check. There is no fee for the use of a notary in the Courthouse.

Registered partners will receive a copy of the registered Declaration of Domestic Partnership, along with a commemorative Certificate of Registered Domestic Partnership form. An optional certified copy may be purchased for an additional $4.00.
